For patients

Notice of Privacy Practices.

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Effective date: October 18, 2026

Who follows this notice

This notice applies to Pontine, Inc. (Pontine Anesthesia Consultants) and the anesthesiologists, nurse anesthetists and staff who provide or bill for anesthesia care on our behalf.

We care for you at surgery centers, medical offices and other facilities that are separate from Pontine. Each facility has its own notice for the records it keeps. We share information with the facility and its staff as needed for your care, and we may work with the facility as one team to coordinate care, safety and quality.

Our website and apps

  • This notice covers the medical and billing information we keep when we provide your anesthesia care.
  • We do not use our website, the Pre-hab Portal or any other app to provide or bill for your care.
  • We do not add anything you enter on our website or in the Pre-hab Portal to your medical record, and we do not combine it with your medical record.
  • Website forms and email are not secure channels for health information. Please do not send medical details through them.
  • Our Website Privacy Notice explains what our website and the Pre-hab Portal collect.

How we may use and share your health information

We may use and share your health information without your written permission for the following purposes.

Treatment

We use your information to evaluate you before surgery, provide your anesthesia and care for you afterward. We share it with your surgeon, the facility's nurses and other clinicians who care for you.

Payment

We use and share your information to bill and get paid by you, your health plan or another payer. For example, we send your health plan the procedure, the length of your anesthesia and your diagnosis codes.

Health care operations

We use your information to run our practice and improve care. Examples include quality and safety reviews, peer review, training, credentialing, audits, legal and compliance work, and business planning.

Business associates

Some services are provided for us by other companies, such as billing and information technology. They receive only the information they need, and a written contract requires them to protect it.

People involved in your care

We may share information with a family member, friend or other person you name who is involved in your care or pays for it, such as the person who drives you home. You can tell us not to. In an emergency, or if you cannot respond, we will use our professional judgment about what is in your best interest.

When the law requires or allows it

  • As required by law, including to the U.S. Department of Health and Human Services when it checks our compliance.
  • Public health and safety, such as reporting diseases, reactions to medicines, problems with medical products, and suspected abuse, neglect or domestic violence.
  • To prevent a serious threat to the health or safety of you, another person or the public.
  • Health oversight, such as audits, inspections and licensing.
  • Lawsuits and legal actions, in response to a court order, or a subpoena when the law's conditions are met.
  • Law enforcement, when the law allows, such as in response to a warrant.
  • Coroners, medical examiners and funeral directors.
  • Organ, eye and tissue donation.
  • Research, when an approved review board has made sure your privacy is protected.
  • Workers' compensation, as the law requires for work-related injuries.
  • Special government functions, such as military command, national security and protective services, and correctional institutions.

Uses that need your written permission

  • We need your written permission to use or share your information for any purpose not described in this notice.
  • We never sell your health information.
  • We do not use your health information for marketing.
  • We do not contact you for fundraising.
  • We do not keep psychotherapy notes.
  • You may cancel a permission you gave at any time by writing to our Privacy Officer. Cancelling does not undo anything we already did based on it.

Extra protections under California and federal law

  • When California law protects your information more than HIPAA does, we follow California law. California's Confidentiality of Medical Information Act often requires your written permission in cases where HIPAA would not.
  • Some information has extra protection. This includes mental health records, HIV test results, genetic test results, substance use disorder treatment records, and reproductive, sexual and gender-affirming health care. California law also limits sharing this information in response to requests from other states.
  • When California law lets a minor consent to their own care, the minor controls the information about that care.

Your rights

To use any of these rights, contact our Privacy Officer. Some requests must be in writing.

  • See and get a copy of your records. You can ask for paper or electronic copies of the medical and billing records we keep about you. We will let you see them within 5 working days and send copies within 15 days of your written request. We may charge a reasonable, cost-based fee for copies. The facility where you had your procedure keeps its own records, so ask the facility for those.
  • Ask us to correct your records. If you think information is wrong or missing, you can ask us to correct it. If we say no, we will explain why in writing, and you can add a statement of disagreement to your record.
  • Choose how we contact you. You can ask us to reach you a certain way, such as at a different phone number or address. We will agree to reasonable requests.
  • Ask us to limit what we use or share. We do not have to agree, except in one case: if you pay for a service in full yourself, you can ask us not to share information about it with your health plan, and we will agree unless the law requires us to share it.
  • Get a list of disclosures. You can ask for a list of the times we shared your information in the 6 years before your request. It leaves out sharing for treatment, payment and operations and a few other cases. One list a year is free.
  • Get a copy of this notice. You can ask for a paper copy at any time, even if you agreed to get it electronically.
  • Choose someone to act for you. A person with your medical power of attorney or your legal guardian can use your rights for you.
  • Be told about a breach. We will notify you if a breach affects your unsecured health information.
  • File a complaint. See below.

Our responsibilities

  • The law requires us to keep your health information private and secure and to give you this notice.
  • We must follow the notice that is in effect.
  • We will not use or share your information other than as described here unless you give us written permission.
  • We may change this notice, and the change will apply to all information we keep. The new notice will be on this page, and we will give you a copy on request.

Questions and complaints

If you believe your privacy rights have been violated, you can complain to us or to the federal government. We will not retaliate against you for filing a complaint.
